Assessment Packages

I currently offer the following assessment packages:

Brief Assessments

Sensory Profile Assessment £150 (remote only)

The Sensory Profile 2 caregiver assessment is a standardised assessment that will indicate how your child responds to sensory stimuli compared to other children their age. The parent or carer fills out an online questionnaire . I will then produce a report and recommendations based on the answers provided. This assessment is done remotely via email. 

The report can be used as evidence to support adjustments at school, request to assess for an EHCP, request for a more detailed sensory assessment, diagnosis of DCD / dyspraxia and a Disability Living Allowance claim. It will not identify provision in EHCP format.

Standard Assessment £350 

This assessment is carried out face to face and will include the following:

  • Initial consultation with parents to understand areas of support required
  • Sensory Profile 2 assessment
  • Any other relevant standardised assessments e.g. PEDI-CAT
  • Parent interview to gather information about the child’s strengths  and challenges
  • Assessment at home or school to observe your child (approximately 1 hour)
  • A report will be provided that addresses your child’s strengths and areas of difficulty and provides recommendations . (The report produced will not be written in EHCP format but may be used to identify need. It will not identify provision in EHCP format). 

Detailed Assessments

These detailed assessments are the most appropriate if you are applying for an EHCP,  your child has an EHCP or you are seeking a diagnosis of DCD. 

The report produced can be used as evidence of areas of need and can contribute towards:

  • Adjustments and reasonable accommodations at school
  • Request to assess for an EHCP
  • Section B (needs section) in the EHCP
  • Section F (provision section) 
  • A diagnosis of developmental co-ordination disorder
  • Disability Living Allowance claim
Sensory Assessment £600

This assessment focuses on sensory needs and how this may be also impacting on physical needs, cognition and learning, communication and interaction and social, emotional and mental  health.

This functional sensory assessment is carried out face to face and will include:

  • Identifying what sensory input your child is seeking, avoiding or sensitive to
  • A home or school visit to explore the impact of the home / school / community environment on your child (approximately 1.5 hours for a home visit and 2 hours for a school visit)
  • A parent interview (approximately 1 hour)
  • Sensory Profile 2 for home and school
  • Considering the role of anxiety and control alongside their sensory profile
  • Communication with school staff if your child is at school
  • Providing advice and education to parents and teachers on understanding and supporting your child’s sensory needs
  • Advising on emotional and sensory regulation and self-advocacy tools. 
  • A report which details the areas above and is written in an EHCP format.  

The assessment is carried out using a standardised assessment questionnaire (Sensory Profile 2), parent interview, observations and activities such as assessing balance, hypermobility, proprioceptive awareness etc. Please note a sensory diet is not included as part of this assessment. There is an additional charge for this.

Dyspraxia / Developmental Co-Ordination Disorder Assessment £600 

During this assessment I will look in detail at your child’s motor skills; if they meet the criteria for a formal diagnosis of DCD and I will provide recommendations to support your child at home and school. (As an Occupational Therapist I am qualified to speak to 3 out of the 4 criteria for a diagnosis of DCD, a GP or pediatrician should confirm the diagnosis).

This assessment include a nursery, school visit or college visit if your child is in an educational setting or a home visit if they are out of school. It will consist of the following:

  • Initial consultation with parents to understand areas of support required
  • Movement ABC-3 assessment (the ‘gold standard’ for assessing for DCD / Dyspraxia
  • Sensory Profile 2 assessment
  • Any other relevant assessments e.g. PEDI-CAT, Anxiety Scales, 
  • Reading of professionals report and EHCP documentation
  • Parent interview to gain information about how your child’s co-ordination difficulties impact them
  • Home visit or school visit to carry out the Movement ABC-3 and a handwriting assessment and to observe your child
  • School visit if appropriate which would involve classroom observation, carrying out the Movement ABC-3 and a handwriting assessment, communication with teaching staff / SENCO (Approximately 2-3 hours)
  • A comprehensive report will be provided with recommendations structured in a way suitable for use in an ECHP.  

EHCP Assessment £600 

During the assessment I will consider all of the following areas – sensory and physical needs, cognitive and learning needs, communication and interaction needs, social, emotional and mental health needs).

This assessment will include a nursery, school visit or college visit if your child is in an educational setting or a home visit if they are out of school. It will consist of the following:

  • Initial consultation with parents to understand areas of support required
  • Sensory Profile 2 assessment
  • Any other relevant standardised assessments e.g. PEDI-CAT, Anxiety Scales, 
  • Reading of professionals report and EHCP documentation
  • Home visit to observe the child completing tasks if appropriate, and to look at the home environment (Approximately 1.5 – 2 hours)
  • School visit if appropriate which would involve classroom observation, assessment of the school environment, communication with teaching staff / SENCO (Approximately 2-3 hours)
  • A comprehensive report will be provided with recommendations structured in a way suitable for use in an ECHP.

Travel Areas

I am based in Gloucester and can travel to your home or your child’s nursery, school or college to do assessments and therapy sessions. The map below indicates the areas where I usually work including whether a travel charge applies (that includes travel time and mileage). If you are outside of this area, please still get in touch and I will see if I can help.

Green zone – travel and mileage time included.

Orange zone – £35 travel charge.

Red zone – £70 travel charge.
